· your general practitioner (doctor)
 

· hospitals / other health professionals caring for you
 

· private dental schemes if you are a member
 

· the Inland Revenue
 

Disclosures only ever disclose the necessary information. If required by law or a court order, personal data may have to be disclosed to a third party. Where possible you will always be informed of these situations.
 

You have the right to access the data that we hold about you and to receive a copy. Requests must be made in writing and will incur an administration fee (available on application).
